- Please use CAPITAL letter in order to fill in the form.
- Please fill in all the form to proceed your application.
- Please attached all the needed documents ( 2 copies of applicant Identity Card, 2 copies of the first guardian Identity Card, 2 copies of second guardian Identity Card, 2 copies of PTPTN Pin Number, 2 copies of applicant CIMB account, 2 copies of SPM certificates and 2 copies of first and second guardian payslip (BRIM receiver))
- Please provide 2 guardians and guardians must be BRIM receiver in order to proceed to applicants.
- After ALL the form filled, the applicants may SCAN and SEND it via E-mail at: Info@unikop.edu.my or also directly send to the Unikop Address.
- You may download PTPTN form on the right side.
- You may find Unikop’s College Address on the right side.
